This form is to be used by those who wish to be considered for nomination or appointment to conference boards and agencies. The deadline for submitting the form is December 31, 2012.

The Alabama-West Florida Conference Cabinet has developed a new format for annual reflection and evaluation that includes (1) Ministry Reflection forms to be completed by both the Pastor and the Staff Parish Relations Committee (SPRC); (2) Appointment Preference forms based on prayerful discernment following thorough reflection upon the entire ministry of the church/charge; and (3) Profile forms for both the pastor and church/charge.
The forms should be completed in this sequence as they were intentionally developed to focus first on actively reflecting on the overall ministry of the church, then to consider appointment preferences in light of the fruitfulness of that ministry, and finally to present a profile of the church and pastor to be available to the Bishop and Cabinet for consideration as appointments are being made in the Spring.
It is our hope and prayer that this process combines several separate evaluations into one, and will focus more on the evaluation of the overall ministry of the congregation and not just the performance of the pastor. It is also hoped that the Pastor and SPRC members will engage in beneficial conversation and goal setting which will truly move the ministry in the direction of making and developing devoted and dedicated disciples of Jesus Christ who partner with Him in bringing about the transformation of the world.
