Clergy Health & Wellness Initiative

Beginning January 1, 2016, the Alabama-West Florida Annual Conference Board of Pension & Health Benefits launched the Health and Wellness Initiative Program. The goal of the program is to encourage our pastors and their dependents to live healthier lifestyles by providing them with personal health information, screening and coaching, thus helping the Conference to reduce and control the cost of health insurance claims.
 
Clergy members on the conference active health insurance plan are required to complete the HealthQuotient and Biometric Screening by August 31, 2016. Dependents, retired clergy and their dependents and employees who are on the annual conference health plan are not required to complete the HQ and Biometric Screening but are welcome and encouraged to participate.
 
The following two steps are required: 
1. To complete your HealthQuotient online, register at www.BeHealthy.com (Click on HealthQuotient in the rotating carousel). If you already have a ‘mybluecross’ account, login the account, if not you will be prompted to create an account.
 
2. A Biometric Screening is required. You may choose from one of the following options to complete the screening.
 
  • Option 1: An on-site Biometric Screening will be available at Annual Conference. Appointments are required. The online appointment scheduler will be available starting May 1st at BeHealthy.com. You will need to login into your ‘mybluecross’ account to schedule your appointment. 
  • Option 2: You may complete your Biometric Screening at a Quest Diagnostic Patient Service Screening Center. Appointments are required. Registration for appointments and scheduling can be completed at My.BluePrintforWellness.com. You will be asked to provide the information below to register.
             Registration Key: AWFUMC2016
             Unique Identifier: First name, Last name, and first 4 digits of DOB or your BCBS Contract Number located on your health
             insurance card.
